Preparing for Your First Visit
Your first visit with us is 100% special, so that’s how we treat it.
This means making sure you feel at home in our office, that we verify all of your important medical information (like that strawberry allergy you rarely mention), and that you are given ample time to get to know us personally and professionally.
During your visit, Dr. Foy will discuss your dental health goals, as well as any aesthetic improvements that you would like to achieve. We believe that working as a team with our patients makes sure you achieve satisfying results.
After reviewing your medical history, x-rays may be taken to determine cavities, gum disease and certain abnormalities. Initial chartings are then performed of your existing restorations, as well as your current dental needs.
Patient education and disease prevention is our highest priority. We not only want to treat your dental needs, but also to teach you how oral health affects your overall medical wellbeing.
We encourage you to save some time by printing and completing the patient forms below in advance of your appointment.
